2021 - 2022 Twin Valley Athletic Registration
For the 2021-2022 school year, the Twin Valley Athletic Department will be utilizing PlanetHS for Athletic registrations. This online registration platform will allow parent/guardians and students-athletes in grades 7 - 12 to register for sports, complete all required athletic department forms and also confirm that you have completed all required steps in order for your student-athlete to start participation in a Fall, Winter or Spring Sport.
IMPORTANT: Physicals must be dated ON OR AFTER June 1st.
Please take a moment to click on and read the following links listed below. These links will provide more information on how to sign your child up for any sports during the 2021-2022 school year, as well as explain how to submit all required forms. It is recommended that the student-athlete create their account FIRST and LINK their account to their parent/guardian by either cell phone number or email. Once the parent/guardian receives the link, they can create their account.
PLEASE MAKE SURE THAT WHEN CREATING YOUR ACCOUNT, YOU SELECT THE CORRECT SCHOOL. STUDENTS IN GRADES 7 AND 8 SHOULD SELECT TWIN VALLEY MIDDLE SCHOOL AND STUDENTS IN GRADES 9 - 12 SHOULD SELECT TWIN VALLEY HIGH SCHOOL.
Students registering for their first sport of the year will need to sign/initial sections 1, 3, 4, 5, and 10, the TVSD Athletic Code and the Student-Athlete Handbook. Students under the age of 13 may not be required to sign all forms.
Parents/Guardians registering their student-athlete for their first sport will need to sign/initial ALL sections and forms.
RECERTIFICATIONS - NOT APPLICABLE FOR THE FALL SEASON
Students and parent/guardians who are competing in a subsequent sport will be required to complete the recertification and complete a Section 8 if required by the Athletic Trainer.
Please note that while all forms are completed and submitted online, you will need to print out Section 5 (Health History) and Section 6 (Physical Form) in order to have a physical. TO PRINT Sections 5 and 6: While on the Section 5 page, click on the "Print/Upload Here" link that is found at the top of the form. This will print all of your answers to Section 5, as well as a blank Section 6 form to take to the doctor.
Below is information for families on how to set up their accounts and complete the athletic registration. We recommend that when possible, all student-athletes utilize their school district issued email when setting up their account.
