Attached is the required Letter of Interest Form that the Twin Valley School District requires non-public school students to complete and submit to the Athletic Department prior to participation on any of our athletic teams.

This form must be submitted and approved before a student is permitted to attend tryouts, practices, or competitions.

Please ensure the completed form is returned to the Athletic Department in a timely manner to avoid any delays in eligibility.
